COVID-19 Vaccination
Appointments are added every few days, so if they are currently full, check back in a day or two.
We offer the Pfizer vaccine, Comirnaty.
Please enter the name and birth date of the person who will be receiving the shot.
Masks are required.
You should expect your appointment to last 20-30 minutes.
You will be asked to provide an email during your registration process. If you do not have an email, just write "NA" in the box.
Vaccine recipients under age 18 must have permission from a parent or guardian, and signatures of both the minor and the parent/guardian must appear on the screening form.
People aged 12 and over should receive 2 doses in the primary series, given 3 weeks apart, followed by an "updated booster" at least 2 months after the 2nd dose or most recent booster.
See CDC for details.
Booster doses do not have to
match your primary series vaccine, it is allowed to mix and match. Find locations handling Moderna or J&J at vaccines.gov.
You may get a flu shot at the same time as a COVID shot - just ask (no appointment needed for flu)!
For 1st and 2nd COVID doses, call us at 715-425-2255.
